This condition involves a bony outgrowth on the distal phalanx (the end bone of the toe), which pushes into the soft tissues and causes uncomfortable friction inside your footwear.
What Causes a Subungual Exostosis?
A subungual exostosis typically develops due to repeated micro-trauma, direct impacts, or excessive pressure around the nail bed. Over time, the underlying bone tissue reacts to this ongoing stress by hypertrophying, forming a bony spur. While this condition is most frequently observed on the big toe, it can also affect the smaller toes.
Clinical Appearance and X-Ray Diagnosis
As the bony outgrowth expands, it pushes the surrounding soft tissue outward. This causes the tip of the toe to bulge and rub against shoes, often leading to thick callus formation and localized pain. To confirm the diagnosis, a podiatrist will examine the foot and usually order X-rays to assess the distal phalanx and evaluate the bone structure.

- Bony spur forming on the distal phalanx underneath or beside the nail bed.
- Triggered by chronic trauma, pressure from tight footwear, or direct injury.
- Causes localized swelling, skin callusing, and tenderness when walking.
- Visible on AP and oblique X-ray views to confirm the bony outgrowth.
